New Client Paperwork

Before your first visit, please download and complete the applicable forms below, and bring them with you to your appointment*. We will also need to make a copy of your insurance card.

If you are unable to bring these forms with you, please arrive at least 15 minutes early to complete them.

*Telehealth-Only Clients: please email completed forms to Office@FamilyGuidanceCenterNH.com along with an image of your insurance card, front and back.
